4. Consent
4.1 How consent is obtained
Before your callback request form can be submitted, you are required to actively confirm your consent by checking a mandatory consent checkbox. By checking this box and submitting the form, you expressly consent to:

5. Third-party disclosure
Your name and phone number may be disclosed to an independent, licensed garage door Service Provider operating in Airdrie, Alberta, for the sole purpose of contacting you to arrange and perform the service you requested. This Service Provider operates independently of Airdrie Garage Door Repair and is solely responsible for the services they provide, including pricing, quality, and any warranties offered.
